Primary Care
Primary care membership includes medication assisted treatment of addiction (MATA) and prenatal care.
* Save $30/m on child memberships when enrolled with an adult.
** Save $50/m with verification of active college enrollment.

Our prenatal package includes a dating ultrasound, routine labs, vitamins, antepartum surveillance, and postpartum follow up visit
When needed, we coordinate your care with specialists to ensure a unified care plan

First time enrollment:
$100 per family if enrolled at the same time
Family enrollment fee covers up to 2 adults and unlimited children
An additional $50 per adult will be assessed if more than 2 adults on the membership
Re-enrollment:
Cancel your membership anytime
If you choose to discontinue membership at any point, you are eligible to re-enroll one time only for $200 per family.
